抗抑郁药预防卒中后抑郁有效性的Meta分析

目的系统评价抗抑郁药预防卒中后抑郁(post stroke depression,PSD)的有效性。方法计算机检索The Cochrane Library(2015年第2期)、PubMed、MEDLINE、EMbase、CNKI和VIP数据库,搜集抗抑郁药预防卒中后抑郁的随机对照试验(RCT),检索时限均为从建库至2015年4月。由2名评价员独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用RevMan 5.3软件进行Meta分析。结果共纳入26个RCT,包括2 190例患者。Meta分析结果显示:与对照组相比,在常规剂量下,使用抗抑郁药能显著降低PSD的发生率[OR=0.24,95%CI(0.17,0.36),P<0.000 01]。按药物种类进行亚组分析结果显示,选择性5-羟色胺(HT)再摄取抑制剂(SSRI)类抗抑郁药物能显著降低PSD的发生率[OR=0.23,95%CI(0.15,0.37),P<0.000 01]。按时间长短进行亚组分析结果显示,无论短期[OR=0.11,95%CI(0.06,0.19),P<0.000 01]、中期[OR=0.31,95%CI(0.21,0.46),P<0.000 01],还是长期[OR=0.30,95%CI(0.19,0.49),P<0.000 01],使用抗抑郁药均能显著降低PSD的发生率。在不良反应方面,试验组和对照组相比,未见差异有统计学意义(P>0.05)。结论抗抑郁药能有效预防PSD的发生,而对患者的生活质量可能没有影响。受纳入研究数量和质量的影响,上述结论尚需开展更多高质量研究予以证实。
