中国不同性别急性心肌梗死患者临床症状及诱发因素的差异分析

目的:探讨中国不同性别的急性心肌梗死患者之间临床症状及诱发因素的差异。方法:分析于2013-01至2014-03期间参加中国急性心肌梗死注册登记(CAMI)研究的患者14 854例,其中男性10 999例(74.0%),女性3 855例(26.0%)。收集患者的临床资料,分析不同性别之间临床症状及诱发因素的差异。结果:持续性胸痛及大汗是我国心肌梗死患者最常见的临床症状,66.4%患者存在持续性胸痛,63.7%的患者存在大汗。与女性患者相比,男性患者胸痛(67.8%vs 62.4%)及大汗(65.8%vs 58.0%)更常见,而女性患者放射痛(36.0%vs 31.0%)及恶心/呕吐(35.6%vs 25.0%)更常见(P<0.05),差异均有统计学意义。19.4%患者存在明确的心肌梗死诱因,其中体力应激(48.5%)最为常见。与女性患者相比,男性患者存在明确的心肌梗死诱因的比例更大(20.9%vs 15.1%),尤其是体力应激(49.5%vs 44.5%)和近期过度不良生活方式(15.4%vs 8.8%)更常见(P<0.05)。结论:胸痛及大汗是最常见的症状。女性患者的胸痛及大汗少于男性,而放射痛及恶心/呕吐较男性常见。仅有1/5的患者存在心肌梗死诱因,其中男性患者更常见。
