老年住院患者营养不良与肌肉减少症及相关因素的分析

摘要
目的通过MNA-SF评估老年科住院患者的营养状况,分析不同营养状况与肌肉减少症及其他相关因素的关系,为患者营养治疗的正确实施提供临床证据。方法选取南京医科大学康达学院第一附属医院自2017年6月至2017年12月入院的65岁以上的老人200例,通过MNA-SF评分将老年科患者分为营养状况正常组、有营养不良风险组及营养不良组,采用人体成分分析仪测定体质指数、骨骼肌量、四肢骨骼肌质量指数,同时测量握力、步速,并收集相关血液指标。结果 200例老年科住院患者中,50. 5%营养状况正常,26. 5%有营养风险,23. 0%存在营养不良;三组肌肉减少症患病率有显著性差异,营养不良组肌肉减少症患病率高于营养风险组和营养状况正常组(P <0. 05);三组血清白蛋白、血清前白蛋白、血红蛋白以及体质指数、骨骼肌量、四肢骨骼肌质量指数、握力、步速等指标差异均有统计学意义(P <0. 05),其中营养不良组<营养风险组<营养状况正常组。结论老年科住院患者中存在较高比例的营养不良风险和营养不良,营养不良的老年患者更容易患肌肉减少症,营养相关指标表达较低,有必要及时进行营养治疗。
