肝素诱发的血小板减少症的诊断和治疗进展

摘要：

肝素诱发的血小板减少症是患者应用肝素后一种少见的、严重的不良反应,其根本原因是患者体内对肝素和血小板4因子的复合物产生了免疫反应,使血小板过度激活。临床特征为血栓栓塞事件发生率增加、血小板被过度消耗引起的血小板减少、血小板减少引起的出血。诊断依据肝素应用史、血小板减少和实验室检查(免疫学检查、血小板功能检查)。肝素诱发的血小板减少症确诊后应停用肝素、应用其他抗凝药物如阿加曲班、比伐卢定、璜达肝癸钠等。
