医院获得性肺炎诊治中面临的困难与挑战——从全国多中心医院获得性肺炎临床调查结果谈起

摘要：

<正>医院获得性肺炎(hospital-acquired pneumonia,HAP)是临床最为常见、危害性最大的医院获得性感染。国外的调查结果表明,在所有医院获得性感染中,HAP的发生率仅次于泌尿系感染,而ICU中HAP的发生率则高居各种医院感染的首位,约占ICU内所有医院感染患者的25%。HAP不仅导致患者住院时间显著延长和住院医疗费用大幅度增加,还往往成为最终导致危重患者死亡的直接原因。美国的调查结果显示,HAP可使患者的平均住院时间延长7～9 d,住院费用增加4万美元以上,累计造成的医疗资源消耗极为惊人。2008年,中华医学会呼吸病学分会感染学组牵头组织10余家大型教学医院联合开展了为期2年多的HAP临床调查,结果显示,呼吸科普通病房和呼吸ICU(RICU)住院患者中HAP的发病率分别为0.88%和15.35%,与欧美
