PNF训练对肩胛骨动力障碍的康复效果

摘要
目的探究4周肩胛带及上肢D2模式本体感觉神经肉促进技术(proprioceptive neuromuscular facilitation,PNF)干预对肩胛骨静态、特殊角度位置及上肢功能的影响。方法选取北京体育大学患有肩胛骨动力障碍(scapular dyskinesis,SD)的学科女生16人,随机分为实验组(T组)和对照组(C组)。T组进行4周上肢PNF干预,C组不做干预,2组受试者实验期间均保持正常的活动。干预前后对肩胛骨平衡角(scapula balance angle, SBA),肩胛侧方滑移测试(lateral scapular slide test,LSST),肩关节功能指标(DASH评分,Y-Balance)测试,进行组内和组间的比较。结果与干预前相比(1) T组SBA明显减小(P=0.01),C组前后无显著性差异;(2) LSST中,T组外展90°位肩胛下角向外滑移减少(P=0.028),C组无显著性差异;(3)肩关节功能指标:①T组和C组干预前后DASH评分均无差异。②T组Y-Balance成绩分数显著提高(P=0.002),且在3个方向:内侧线(P=0.005)、外上线(P=0.008)、外下线(P=0.003)测试距离均有增加。结论 4周PNF干预可有效改善肩胛骨静态和动态位置:表现为肩胛骨平衡角减小,肩外展90°时肩胛骨外移减少;肩胛骨位置的改善能有效提高上肢功能。
