非痴呆性血管性认知功能障碍脑梗死患者rTMS治疗的临床研究

摘要
目的观察重复经颅磁刺激(rTMS)治疗非痴呆性血管性认知功能障碍(VCIND)脑梗死患者认知功能改善的有效性、安全性。方法将VCIND脑梗死患者分为rTMS组和对照组,rTMS组另给予低频r TMS刺激。分别于治疗前及各疗程后进行MMSE、Moca、NIHSS、ADL、Hachinski、Fazekas,CHIPS及HAMD-17等量表评分。比较2组差异及进行相关因素分析。结果 12组基线比较差异无统计学意义(P>0.05)。2治疗3月后,rTMS组MMSE、Moca评分均较治疗前明显升高。rTMS组Moca值升高程度明显高于对照组(P<0.01)。3rTMS组Moca值随着疗程逐渐递增,于第三疗程明显升高(P<0.01)。4治疗3月后,rTMS组Moca各单项目中视空间与执行能力、语言及延迟记忆得分均较治疗前及对照组明显升高(P<0.01,<0.05)。5治疗3月后r TMS组中不伴抑郁患者Moca评分升高程度明显高于伴抑郁患者;首次卒中Moca评分升高程度明显高于多次卒中。结论 rTMS治疗能明显促进VCIND脑梗死患者认知功能恢复。
