叶酸受体阳性CTC检测在恶性肺部结节辅助诊断中的应用

摘要
目的评价靶向叶酸受体(folate receptor, FR)阳性循环肿瘤细胞(circulating tumor cell, CTC)检测方法在临床肺部结节良恶性诊断中的实用价值。方法选取2017年5月至2018年4月本院收治的肺部结节筛查患者288例,采用靶向FR CTC技术检测外周血中CTC含量;结合患者术后的病理检查结果,分析靶向FR CTC检测在肺部恶性肿瘤辅助诊断中的准确性、可行性。结果以8.7 Folate Units/3 mL为cut-off值,136例肺部恶性肿瘤的CTC水平显著高于152例肺部良性疾病患者(P<0.05)。136例肺部恶性肿瘤FR阳性CTC 107例,152例良性病变结节FR假阳性CTC 44例,总体上靶向FR CTC检测技术对肺部恶性肿瘤的检测灵敏度为78.7%,特异性为71.1%。特别是在Ⅰ期肺癌灵敏度达到76.9%,9例原位肺癌CTC检测均呈阳性,与肿瘤标志物、CT检查等比较,CTC检测对早期肺癌具有较高的辅助诊断价值。结论靶向FR CTC检测可以应用于肺部结节良恶性的筛查,对肺部恶性肿瘤的早发现有较好的辅助诊断价值。
