缺血性脑卒中后血管性认知障碍的流行病学调查

摘要
目的研究中国湖南长沙地区缺血性脑卒中后血管性认知障碍的患病率及危险因素。方法采用长沙版蒙特利尔认知评估量表(MoCA)和简易智能精神状态检查量表(MMSE)对689例缺血性脑卒中患者进行认知功能评估,并且参照北京天坛医院发表的血管性认知障碍(VCI)及其亚型诊断标准,将患者分为认知正常(CN)组和VCI组,登记患者的人口学资料、血管危险因素、脑卒中的严重程度和个人生活习惯。结果 (1)缺血性脑卒中后VCI的患病率为41.8%;(2)单因素分析显示缺血性脑卒中后VCI的发生与年龄、文化程度、饮酒、尿失禁、卒中后癫痫、球麻痹、运动功能障碍和阅读相关;(3)多因素Logistic回归分析显示:年龄,文化程度,饮酒,尿失禁,运动障碍和阅读是缺血性脑卒中后VCI的影响因素。结论在缺血性脑卒中患者中VCI的患病率高,且年龄、文化程度、饮酒、尿失禁、运动障碍和阅读是其独立的影响因素。
