我国60岁以上老年人群肌少症患病率及相关因素调查

摘要
目的分析我国社区居住老年人发生肌少症的患病率, 并探讨与其发生的相关因素。方法资料来源于2015年我国28个省、自治区和直辖市《中国健康与养老追踪调查(CHARLS)》全国开放的数据库, 以2014年亚洲肌少症工作组(AWGS)推荐标准, 对我国社区60岁及以上7 584人进行了肌少症标准检查的居民, 进行横断面调查。以男女性别分组, 比较一般社会人口学特征和生活习惯, 对肌少症患病率进行分层分析和采用Logistic回归进行多因素分析。结果我国老年人群肌少症的患病率6.4%(95%CI:5.9~7.0)。在肌少症的患病率分层分析中, 男性9.9%(95%CI:9.0~10.9)明显高于女性3.0%(95%CI:2.4~3.5);农村7.2%(95%CI:6.5~7.9)明显高于城镇4.3%(95%CI:3.4~5.2);吸烟者8.8%(95%CI:7.6~10.0)明显高于不吸烟者4.2%(95%CI:3.6~4.8)。多因素Logistic回归表明, 男性(OR=5.368, 95%CI:4.126~6.985)、增龄(OR=1.191, 95%CI:1.172~1.210)是肌少症的危险因素。此外, 身体疼痛(OR=2.181, 95%CI:1.695~2.673)、长期饮酒(OR=1.426, 95%CI:1.057~1.923)、受教育程度低(OR=2.875, 95%CI:1.577~5.241)、腰围增加(OR=0.982, 95%CI:0.973~0.990)、最大呼气流量减少(OR=0.995, 95%CI:0.994~0.997)、胱抑素C增加(OR=2.088, 95%CI:1.247~3.495)均与肌少症的发生有明显的相关性(均P<0.05)。结论我国社区老年人中肌少症患病率较高, 且肌少症的发生与年龄、性别、受教育程度、腰围、饮酒等密切相关。
