脊髓型颈椎病减压术后早期脊髓MRIT2WI信号改变对临床疗效的影响

摘要
目的分析接受颈椎减压手术治疗的脊髓型颈椎病患者术后早期脊髓MRI T2加权像高信号改善情况与术后临床疗效的关系,探讨其对术后临床疗效的预测作用。方法将纳入研究的患者按照颈椎减压术后1年T2加权像高信号改善情况分为恢复组与未恢复组。收集数据包括性别、年龄、病程、手术入路、减压节段、C2~7前凸角、术前改良日本骨科协会(modified Japanese orthopaedic association,mJOA)评分、术前疼痛视觉模拟评分(visual analogue scale,VAS)、术后mJOA改善率、术后VAS评分,并进行统计学比较。结果 T2加权像高信号恢复组纳入50例,未恢复组纳入92例。恢复组mJOA术后1年(14.6±1.2)分,术后2年(14.6±1.2)分,术后3年(14.4±1.2)分,术后4年(14.5±1.2)分,未恢复组mJOA术后1年(11.5±2.6)分,术后2年(11.5±2.5)分,术后3年(11.5±2.6)分,术后4年(11.4±2.6)分,各随访时间两组术后mJOA之间差异有统计学意义(P<0.001);恢复组mJOA改善率术后1年(52.5±21.9)分,术后2年(51.9±21.8)分,术后3年(48.1±22.2)分,术后4年(49.8±22.4)分,未恢复组mJOA改善率术后1年(33.8±17.4)分,术后2年(33.7±17.8)分,术后3年(33.7±17.8)分,术后4年(32.5±17.6)分,各随访时间两组术后mJOA改善率之间差异有统计学意义(P<0.001);恢复组上肢VAS评分术后1年(24.8±11.1)分,术后2年(24.7±8.1)分,术后3年(24.8±8.1)分,术后4年(24.6±10.5)分,未恢复组VAS评分术后1年(42.6±14.9)分,术后2年(42.9±13.9)分,术后3年(41.8±13.1)分,术后4年(41.6±12.6)分,各随访时间两组术后上肢VAS评分之间差异有统计学意义(P<0.001)。结论脊髓型颈椎病患者行颈椎减压术治疗后1年,脊髓T2HSC改善情况可能在预测患者术后4年的临床疗效中有重要作用。术后1年脊髓T2HSC明显恢复的患者术后运动及感觉功能恢复情况明显优于未恢复的患者。颈椎减压手术治疗效果在术后1年内恢复最佳,并在1年后临床疗效较为稳定。
