<正>患者女,43岁,主因"双手指关节反复肿痛7年,加重2周"于2010年4月到我院就诊。患者7年前无明显诱因出现双手小关节肿胀,疼痛,就诊于我院风湿科,实验室检查:类风湿因子(RF)1:80 U/ml,双手X线提示双手指骨端骨质疏松,依据美国风湿病学会(ACR)1997年修订的类风湿关节炎(rheumatoid arthritis,RA)分类标准诊断为"RA",予口服非甾体抗炎药(NAISDs)、帕夫林,来氟米特等药物,症状缓解。用药3个月后生化提示肝功能损坏(丙氨酸转氨酶>200 U/L总胆红素27μmol/L),停用上述药物1个月后肝功能恢复正常,之后关节肿痛反复发作,换用甲氨蝶呤,羟氯喹等免疫抑
