摘要：

<正>在全球老龄化加速发展的背景下,我国人口老龄化形势日益严峻。第七次全国人口普查数据显示,我国60岁及以上人口为2.64亿,比例达到18.70%。老龄化带来最突出的问题是健康与疾病问题,老年人存在很高的共病患病率。根据世界卫生组织(WHO)的定义,共病(Multimorbidity)是指同一个体同时存在多种健康问题,通常是慢性病,需要复杂、持续的照护[1]。老年共病使医疗决策变得复杂和困难,经常会造成多重用药、治疗不连续、过度医疗等医源性问题,存在潜在的药物治疗风险[2]。
