3D打印辅助高位胫骨截骨治疗内侧室膝骨关节炎

摘要
[目的]评价个性化3D打印截骨模具辅助高位胫骨截骨治疗膝内侧间室骨关节炎的临床效果。[方法]回顾性分析2017年5月—2019年5月本院采用高位胫骨截骨治疗的膝内侧间室骨关节炎120例患者的临床资料,依据术前医患沟通结果,62例采用3D打印辅助截骨(3D组),58例采用常规截骨(常规组)。比较两组围手术期、随访及影像学资料。[结果]两组患者手术均顺利完成,无严重并发症。3D组手术时间、术中出血量、术中射线曝光时间和住院时间均显著优于常规组(P<0.05)。所有患者均随访12~32个月,平均(15.52±3.41)个月。3D组恢复下地行走时间和完全负重活动时间均显著早于常规组(P<0.05)。与术前相比,末次随访时两组患者VAS评分均显著减少(P<0.05),HSS评分和膝伸屈ROM均显著增加(P<0.05)。末次随访时3D组VAS评分、HSS评分和ROM均优于常规组(P<0.05)。影像方面,与术前相比较,末次随访两组患者FTA和MPTA均显著改善(P<0.05),PTS无显著改变(P>0.05)。末次随访时,3D组的FTA和MPTA均优于常规组(P<0.05)。3D组愈合时间显著早于常规组(P<0.05)。[结论]在膝关节内侧间室骨关节炎的患者中应用个体化3D打印截骨模具辅助可显著提升高位胫骨截骨患者的治疗效果。
