新生儿急性肾损伤诊断及生物标记物研究进展

摘要
新生儿急性肾损伤(acute kidney injury,AKI)发生率高。围产期多种因素可引起新生儿AKI。新生儿AKI可导致慢性肾脏病等近期和远期不良预后。早期识别新生儿AKI并给予及时治疗,可显著改善患儿预后。但由于新生儿特殊的病理生理特点,成人和儿童AKI的诊断标准并不适用于新生儿,使得新生儿AKI的诊断困难,现有的诊断标准也不统一。近年来,新的肾损伤生物标记物不断发现,如中性粒细胞明胶酶相关脂质运载蛋白、肾损伤分子-1和白细胞介素-18等。这些生物标记物对新生儿AKI的早期识别、提高诊断效率及判断预后等具有重要价值,但尚需要进一步研究,评估其影响因素、参考范围及临床意义。
