摘要：

目的探讨登革热(dengue fever , DF)患者病毒载量和特异性IgM抗体在疾病过程中的动态变化规律及其与疾病严重程度的相关性。方法收集1 140例DF患者(其中轻症1 050例, 重症90例)发病第1～10天的血清1 508份, 用荧光定量PCR法检测病毒载量, ELISA法检测特异性IgM抗体。结果 DF患者病毒载量在发病第1天时高达108拷贝/ml, 随病程进展逐渐下降, 第10天时降至103拷贝/ml, 重症患者病毒载量在发病的第5～7天高于轻症患者, 差异有统计学意义;核酸阳性率也随病程进展逐渐下降, 但至第10天时, 仍有约40%的阳性率, 重症患者核酸阳性率在第6天和第8天高于轻症患者, 差异有统计学意义。IgM抗体最早可在发病的第2天检测到, 随病程进展逐渐上升, 第10天时水平仍在上升;轻症患者IgM抗体水平在发病第4天高于重症患者, 差异有统计学意义。IgM抗体阳性率在发病第6天时达到95%, 轻症患者的IgM抗体阳性率在发病第5～6天显著高于重症患者, 差异有统计学意义。结论登革热退热期的高病毒载量和低IgM抗体水平与疾病的严重性密切相关。
