β-blockers as fourth-line therapy for hypertension: stay the course

摘要
In the provocative article published in a recent issue ( 1), Henry Krum argues that beta-blockers should remain as the first-line therapy in the management of hypertension. Before we go about reversing any of the national and international guidelines stance on beta-blocker for hypertension, it is essential to scrutinise closely the evidence suggested by Krum for such a change. The arguments listed include evidence of harm only from older generation beta-blockers, possible beneficial effect in the young and in patients with comorbidities.
