Ischaemic conditioning is an endogenous form of cardioprotection which can be elicited by cycles of brief ischaemia and reperfusion to the heart directly or to an organ or tissue away from the heart. A number of proof-of-concept clinical studies have shown beneficial effects with ischaemic conditioning in the settings of cardiac bypass surgery, elective PCI and PPCI, with reduced myocardial injury and preservation of cardiac function. Whether ischaemic conditioning can actually improve clinical outcomes in CABG and PPCI patients is currently being investigated in several large multicentre randomised clinical trials. As a result, in the next couple of years we should know whether ischaemic conditioning could benefit patients with IHD in terms of reducing morbidity and mortality and potentially change clinical practice. © 2015, BMJ Publishing Group.
