Exo-hsacircRNA0056616:肺腺癌淋巴结转移的潜在标志物研究

摘要
目的探讨CXCR4表达与肺腺癌淋巴结转移的关系;筛选与CXCR4相关的circ RNA,寻找判定肺腺癌淋巴结转移的生物学标志物。方法1、收集肺腺癌手术患者组织标本,通过Wsternblot技术检测肺腺癌组织标本CXCR4的蛋白表达,分析肺腺癌组织标本CXCR4的蛋白表达水平与淋巴结转移的关系。2、通过慢病毒介导的RNA干扰构建CXCR4稳定低表达PC-9细胞,通过软琼脂克隆形成实验、流式细胞术、CCK8、transwell实验、细胞划痕等实验检测CXCR4表达的变化对肺腺癌PC-9细胞的增殖与转移能力的影响。3、在circbase、circ RNABase、Circ2Traits等数据库上搜索与CXCR4相关的circ RNA,同时运用实时荧光定量PCR技术在PC-9细胞及外泌体中检测沉默CXCR4表达前后与CXCR4相关的circ RNA的表达差异。4、收集临床肺腺癌手术患者血液标本;应用实时荧光定量PCR技术检测肺腺癌血浆exo-hsacirc RNA0056616的表达,分析肺腺癌患者血浆exo-hsacirc RNA0056616的表达与组织CXCR4蛋白表达的相关关系。5、通过分析肺腺癌患者的临床病理资料,探讨肺腺癌患者血浆exo-hsacirc RNA0056616表达与淋巴结转移之间的关系。结果1、肺腺癌淋巴结转移患者的癌组织标本CXCR4蛋白表达量水平高于肺腺癌淋巴结未转移患者,差异具有统计学意义(P=0.001)。2、靶向沉默PC-9细胞CXCR4的表达,软琼脂克隆形成实验表明,PC-9/CXCR4-sh RNA细胞组克隆数量与阴性对照PC-9/sh Ctrl组及空白对照PC-9细胞组比较明显减少,差异具有统计学意义(P<0.001);CCK8实验结果显示,PC-9/CXCR4-sh RNA细胞组增殖率低于阴性对照PC-9/sh Ctrl组及空白对照PC-9细胞组,差异有统计学意义(P<0.01);transwell实验表明PC-9/CXCR4-sh RNA细胞组与阴性对照PC-9/sh Ctrl组及空白对照PC-9细胞组相比,细胞穿膜数量明显减少,差异具有统计学意义(P<0.001);细胞划痕实验表明,PC-9/CXCR4-sh RNA细胞组与阴性对照PC-9/sh Ctrl组及空白对照PC-9细胞组相比,细胞迁移率降低,差异具有统计学意义(P<0.001)。3、在circbase、circRNABase、Circ2Traits等数据库上搜索到与CXCR4相关的circ RNA是hsacirc RNA0056615、hsacirc RNA0056616、hsacirc RNA0117403。4、靶向沉默PC-9细胞CXCR4的表达,与CXCR4相关的circ RNAhsacirc RNA0056616在PC-9/CXCR4-sh RNA组细胞及外泌体中的表达量较阴性对照PC-9/sh Ctrl组及空白对照PC-9组中细胞及外泌体中的表达量减少,差异具有统计学意义(P<0.001)。5、经对数转换后,肺腺癌组织CXCR4的蛋白表达及血浆exo-hsacirc RNA0056616表达量的Pearson积矩相关系数r=-0.979(P≤0.05)。6、经Wilcoxon秩和检验分析肺腺癌T1/T2期的患者血浆exo-hsacirc RNA0056616表达量高于肺腺癌T3/T4期的患者;肺腺癌M0期的患者血浆exo-hsacirc RNA0056616表达量高于肺腺癌M1期的患者;肺腺癌I-II级的患者血浆exo-hsacirc RNA0056616表达量高于肺腺癌III-IV的患者(P值分别为0.002、0.004、0.001),与肺腺癌患者的年龄、性别、肿瘤大小、组织分型无关(P值分别为0.869、0.765、0.122、0.205)。Spearman秩相关分析显示,血浆exo-hsacirc RNA0056616的表达水平与肺腺癌患者的T分期、M分期、TNM分级的Spearman相关系数rs分别为-0.318(P=0.003)、-0.308(P=0.003)、-0.466(P=0.001),与肺腺癌患者的肿瘤大小、组织分型的Spearman相关系数rs分别为-0.232(P=0.060)、0.154(P=0.207)。7、实时荧光定量PCR结果表明,肺腺癌有淋巴结转移的患者的血浆exo-hsacirc RNA0056616的表达量低于肺腺癌淋巴结未转移患者,差异具有统计学意义(P<0.05)。exo-hsacirc RNA0056616在90例血浆标本中诊断出肺腺癌淋巴结未转移的ROC曲线显示AUC为0.812(标准误0.047,95%可信区间0.720~0.903),最大约登指数0.602,灵敏度0.792,特异度0.810,阳性预测值0.826,阴性预测值0.773,对应区分界值0.394。结论1、CXCR4的表达水平与肺腺癌淋巴结转移有关,且有淋巴结转移患者CXCR4的表达量高于无淋巴结转移患者。2、血浆ex-hsacirc RNA0056616的表达量与肺腺癌组织CXCR4蛋白的表达水平存在负相关关系,肺腺癌患者淋巴结有无转移与肺腺癌组织CXCR4蛋白表达水平存在正相关关系,且肺腺癌组织CXCR4的蛋白表达水平越高,则血浆ex-hsacirc RNA0056616表达量越低。3、利用exo-hsacirc RNA0056616可以较好区分肺腺癌淋巴结转移患者与未转移患者,当截断点取0.394,灵敏度0.792,特异度0.810,阳性预测值0.826,阴性预测值0.773,血浆exo-hsacirc RNA0056616很可能是肺腺癌淋巴结转移的潜在生物学标志物。
