双能CT虚拟去钙成像对腰椎间盘退变诊断价值的评估

摘要
目的以MRI腰椎间盘退变程度主观分级及T2值为参考标准,从主观视觉评估和客观定量评估两方面评价双能CT(Dual-energy computed tomography,DECT)虚拟去钙技术(Virtual noncalcium,VNCa)对腰椎间盘退变的诊断价值。方法共招募志愿者51名,均完成了腰椎DECT、腰椎MRI常规序列及MRI集合序列(Magnetic resonance image complication,MAGiC)检查,CT与MRI检查间隔时间不超过两周。医师1与医师2根据改良的Pfirrmann分级系统(Modified Pfifirrmann grade,mPG)通过T2WI图像对所有椎间盘进行退变程度分级,意见不一致时,协商达成一致结果;随后在由MAGiC序列重建得到的T2-mapping图像上绘制感兴趣区(Region of interest,ROI)以测量髓核和纤维环的T2值,ROI1代表纤维环,ROI3代表髓核,取两名医师结果的平均值用作统计分析。医师3与医师4根据彩色编码的VNCa图像上髓核衰减情况独立将椎间盘分为三种不同视觉类型,并在彩色编码的VNCa图像上绘制ROI以测量髓核及纤维环的VNCa CT值和常规CT(regularCT,rCT)值,仍用ROI1代表纤维环,ROI3代表髓核。采用Kappa检验评估观察者间VNCa视觉分型的一致性。采用组内相关系数(Intraclass correlation coefficient,ICC)分别评估观察者间T2值、VNCa CT值以及rCT值的一致性。采用Spearman秩相关分别比较VNCa视觉分型与mPG分级、mPG分级与VNCa CT值、mPG分级与rCT值、VNCa视觉分型与T2值,以及VNCa CT值、rCT值与T2值的相关性。采用Fisher精确检验分析不同VNCa视觉分型与mPG分级间有无显著差异。根据以mPG或VNCa视觉分型分组后各组数据符合正态分布及方差齐性与否,选择ANOVA、Kruskal-Wallis或Welch检验不同mPG分级间VNCa CT值及rCT值有无显著差异以及不同VNCa视觉分型间T2值有无显著差异。当P<0.05时,认为差异有统计学意义。结果1共纳入225个腰椎间盘,各VNCa视觉分型间mPG分级差异有统计学差异(P均<0.001),且VNCa视觉分型与mPG分级呈显著正相关(rs=0.752、0.701,P均<0.001)。2 mPG分级与髓核VNCa CT值呈显著正相关(rs=0.536、0.474,P均<0.001),除mPG1级与2级、3级之间,4级与5+级之间髓核VNCa CT值无明显差异外(医师3:P=0.546、0.136、0.101,医师4:P=0.999、0.230、0.687),其余各组间VNCa CT值差异均有统计学意义;各mPG分级髓核rCT值无统计学差异(P=0.432、0.116)。3 VNCa视觉分型与髓核T2值呈显著的负相关(rs=-0.631、-0.636,P均<0.001),不同VNCa视觉分型之间髓核的T2值差异均有统计学意义(P均<0.001)。4髓核VNCa CT值与髓核T2值呈显著负相关(rs=-0.424、-0.437,P均<0.001)。5纤维环rCT值与mPG分级呈显著负相关(rs=-0.416、-0.462,P均<0.001),mPG2级与4级、5+级之间,以及mPG3级与5+级之间纤维环rCT值存在显著差异(P均<0.05),其余各组间纤维环rCT值均无明显差异。不同mPG分级对应的纤维环VNCa CT值无显著差异(医师3:F=1.816,P=0.127;医师4:F=0.916,P=0.455),且纤维环VNCa CT值与mPG分级之间无明显相关性(医师3:rs=-0.115,P=0.86;医师4:rs=-0.066,P=0.323)。结论1 DECT VNCa技术可用于评估腰椎间盘退变,VNCa视觉分型可作为MRI mPG分级的替代方法,用于对有MRI检查禁忌症的患者腰椎间盘退变程度的评估。2腰椎间盘髓核VNCa CT值可作为评估腰椎间盘退变的客观参数,辅助评估腰椎间盘退变。图8幅;表4个;参156篇。
