TCF7L2和IGF2BP2基因多态性与新疆维、哈两族2型糖尿病易感性的关系

摘要
目的:2型糖尿病是一种由遗传因素和环境因素相互作用引起的复杂的慢性疾病。流行病调查研究显示新疆维吾尔族和哈萨克族人群2型糖尿病的发病率差别很大(14.00%比5.37% , P<0.005),但对饮食等环境因素分析结果不能解释这种差异。近来有研究报道TCF7L2和IGF2BP2基因变异与2型糖尿病的发生关系密切,本课题研究是否维吾尔族和哈萨克族2型糖尿病发病差异与TCF7L2和IGF2BP2基因多态性有关。方法:用AMRS-PCR法对TCF7L2 rs 12255372进行基因分型,用HRM法对TCF7L2 rs7903146和IGF2BP2 rs4402960基因分型。分析TCF7L2和IGF2BP2基因多态性与维吾尔族和哈萨克族2型糖尿病遗传易感性的关系。比较维、哈两族人群中TCF7L2和IGF2BP2基因变异与糖代谢相关指标的关系,分析TCF7L2和IGF2BP2基因变异影响2型糖尿病易感性的可能机制。分析两民族糖、脂以及肥胖的关联性,为其T2DM的预防和治疗提供依据。结果:rs12255372变异与维吾尔族空腹血清胰岛素(FINS)、稳态模型胰岛素抵抗指数(HOMA-IR)以及总胆固醇(TC)浓度升高有关;rs7903146基因突变与哈萨克族LDL的降低有关;rs4402960基因突变与维吾尔族LDL的升高以及哈萨克族体重指数和甘油三酯的升高有关。维吾尔族TC、TG、LDL浓度与FINS、FBG和HOMA-IR均呈正相关(P<0.05);哈萨克族FBG、INS和HOMA-IR均与WC呈正相关(P<0.05),INS和HOMA-IR与BMI呈正相关。结论: TCF7L2基因变异与维吾尔族胰岛素抵抗有关,从而使T2DM的易感性增加; TCF7L2基因变异与哈萨克族T2DM的易感性有关,但本课题未发现TCF7L2基因变异引起T2DM的确切机制;TCF7L2 rs1225372基因变异可能是维、哈两族T2DM患病差异的原因之一;未发现IGF2BP2基因变异与维、哈两族T2DM的发生有关,但IGF2BP2基因变异与两民族肥胖和脂代谢紊乱有关;维吾尔族糖脂代谢密切相关,哈萨克族糖代谢与肥胖的关系密切。
