IL-23R基因多态性与四川地区汉族人群类风湿关节炎的关联研究

摘要
目的:探讨白介素23受体(interleukin-23 receptor,IL-23R)基因4个位点(rs1004819、rs7517847、rs10489629、rs2201841)单核苷酸多态性(single nucleotide polymorphisms,SNPs)与四川地区汉族人群类风湿关节炎(rheumatoid arthritis,RA)发病易感性及临床特点的相关性,从遗传学角度进一步明确IL-23R在RA发病机制中的作用。方法:采用单碱基延伸法对192例RA患者及192例健康对照者血液DNA中IL-23R基因4个位点SNPs进行基因分型,计算各位点SNPs基因型分布及等位基因频率,比较RA组与健康对照组基因型及等位基因频率差异。通过对RA组进行类风湿因子(rheumatoid factor,RF)、抗环瓜氨酸肽抗体(anti-citrullinated peptide antibody,ACPA)分层分析,进一步探讨IL-23R基因多态性与RA临床亚组之间的关系,同时结合RA患者性别、年龄、血沉(erythrocyte sedimentation rate,ESR)、C反应蛋白(C reactive protein,CRP)、疾病活动度28评分(disease activity score28,DAS28)等相关指标,比较RA组各SNPs位点基因型之间指标的差异。利用Haploview 4.2软件进行连锁不平衡(linkage disequilibrium,LD)构建和单倍体型分析。结果:RA组和对照组IL-23R基因4个位点SNPs基因分型结果均符合哈迪-温伯格平衡(Hardy-Weinberg equilibrium,HWE)(p>0.05)。rs10489629基因型GG及等位基因G频率在RA组均高于对照组,差异有统计学意义(P=0.027,P=0.006),携带基因型GG患者发生RA的优势比(odds ratio,OR)值为1.68(P=0.027,OR=1.68,95%CI=1.09-2.61),携带等位基因G患者发生RA的OR值为1.55(P=0.006,OR=1.55,95%CI=1.11-2.17)。其余3个位点(rs1004819、rs7517847、rs2201841)基因型及等位基因频率分布在两组间均无统计学差异(p>0.05)。rs10489629等位基因G频率在RF+RA和ACPA+RA与健康对照组相比,差异亦有统计学意义(P=0.002,P=0.032),等位基因G携带者患RF+RA和ACPA+RA的OR值分别为1.59和1.68(P=0.002,OR=1.59,95%CI=1.17-2.18;P=0.032,OR=1.68,95%CI=1.21-2.46)。RA患者组IL-23R基因各位点不同基因型间性别、年龄、ESR、CRP及DAS28评分上差异均无统计学意义(p>0.05)。单倍体型CTAC的频率在RA患者组明显低于正常对照组(Pc=0.001,OR=0.27,95%CI=0.16-0.47),而单倍体型CGGT频率在RA患者组高于对照组(Pc=0.014,OR=3.27,95%CI=1.52-7.04)。其他单倍体型频率在两组间均无统计学差异(Pc>0.05)。结论:1.IL-23R基因位点rs10489629多态性可能与四川地区汉族人群RA易感性相关,基因型GG和等位基因G可能为RA发病风险因素。2.IL-23R基因位点rs10489629可能与RF+RA和ACPA+RA存在易感相关,等位基因G可能是RF+RA和ACPA+RA发病风险因素。3.IL-23R基因多态性与RA患者性别、年龄、血沉、CRP及DAS28没有明显关联。4.单倍体型CTAC对RA发病可能起保护作用,单倍体型CGGT可能增加了RA发病风险。
