化瘀通阳方对溃疡性结肠炎大鼠IL-23/JAK-STAT信号通路影响的实验研究

摘要
目的:观察化瘀通阳方对溃疡性结肠炎大鼠白细胞介素-23(IL-23)含量及JAK-STAT信号通路的影响,以探究其作用机制。方法:将32只健康Wistar大鼠按照随机数字表法分为空白对照组8只,模型复制组24只,并采用5%葡聚糖硫酸钠(DSS)溶液灌胃结合自由饮用方式建立溃疡性结肠炎大鼠模型,7天后随机处死空白对照组2只大鼠及模型复制组6只大鼠,立即剖取大鼠结肠组织,通过观察其肠管长度、黏膜溃疡情况、组织病理变化等验证造模是否成功。造模成功后第2天,按照随机数字表法将模型复制组大鼠分为化瘀通阳组、美沙拉嗪组和模型组,每组各6只大鼠。各组分别给予相应药物灌肠干预,化瘀通阳组予化瘀通阳方(0.54g/m L)灌肠,美沙拉嗪组予美沙拉嗪灌肠液(0.036g/m L)灌肠,模型组及空白对照组均予生理盐水灌肠。各组给药剂量均为2m L/只/日,每日1次,持续10d。实验期间采用DAI评分评价大鼠一般情况。治疗结束后采用颈椎脱臼法处死全部大鼠,并立即剖取大鼠结肠,比较各组大鼠肠管长度变化,采用CMDI评分评价大鼠结肠组织形态损伤程度,采用HE染色观察结肠组织病理变化,采用ELISA法检测结肠组织中IL-23含量,采用Western Blot法检测结肠组织中JAK1、TYK2、STAT1、STAT4蛋白的表达。结果:DAI评分结果显示:造模期间,较空白对照组,模型复制组大鼠DAI评分呈明显上升趋势;治疗结束后,较模型组,化瘀通阳组和美沙拉嗪组大鼠DAI评分明显下降。大鼠结肠长度结果显示:较空白对照组,模型组大鼠肠管短缩明显(P<0.01);化瘀通阳组和美沙拉嗪组大鼠肠管短缩情况与模型组大鼠比较有明显改善(P<0.01)。大鼠结肠组织CMDI评分结果显示:较空白对照组,模型组大鼠结肠黏膜表面粗糙,肠壁增厚,可见溃疡形成,黏膜损伤严重(P<0.01);较模型组,化瘀通阳组和美沙拉嗪组大鼠结肠组织损伤均得到明显改善(P<0.01),且两组间差异无统计学意义(P>0.05)。HE染色及TDI评分结果显示:较空白对照组,模型组大鼠结肠组织损伤程度最重,可见固有层腺体结构破坏,隐窝结构紊乱,炎性细胞浸润达黏膜下层(P<0.01);较模型组,化瘀通阳组和美沙拉嗪组大鼠结肠黏膜固有层腺体排列基本整齐,可见少量炎性细胞(P<0.01),且两组间差异无统计学意义(P>0.05)。ELISA检测结果显示:较空白对照组,模型组大鼠结肠组织中IL-23含量明显增高(P<0.01);较模型组,化瘀通阳组和美沙拉嗪组大鼠结肠组织中IL-23含量降低(P<0.01),且两组间差异无统计学意义(P>0.05)。Western Blot检测结果显示:较空白对照组,模型组大鼠结肠组织中JAK1、STAT1、STAT4蛋白均呈高表达;较模型组,化瘀通阳组和美沙拉嗪组大鼠结肠组织中JAK1、STAT1蛋白表达明显下调(P<0.01),STAT4蛋白表达降低(P<0.05),TYK2蛋白变化不明显(P>0.05)。结论:1.UC大鼠结肠组织中IL-23与JAK1、STAT4蛋白表达呈正相关关系,且JAK1蛋白与STAT1、STAT4蛋白表达亦呈正相关关系,提示IL-23/JAK-STAT信号通路可能为UC发病过程中的一条新途径;2.化瘀通阳方可能通过降低IL-23水平,调控JAK-STAT通路,下调JAK1、STAT1、STAT4蛋白表达,从而减轻肠道炎症反应和黏膜损伤,对UC病变处发挥作用。
