牛津认知筛查量表的汉化修订及在脑卒中患者中的信度和效度研究

摘要
目的:1.通过翻译及修订牛津认知筛查量表(Oxford Cognitive Screen, OCS)英文原版,形成普通话版牛津认知筛查量表(Putonghua Version of Oxford Cognitive Screen, OCS-P)。2.对OCS-P进行临床实地测试,探讨其在我国脑卒中患者中应用的信度和效度。方法:在获得OCS原量表作者的授权后,根据国际FACIT翻译原则对量表进行翻译和汉化形成OCS-P的初量表。邀请相关领域的5位专家组成专家评审小组,进行OCS量表关于等义性、流畅性、关联性、代表性等内容效度评审,针对有争议的翻译条目进行讨论,并通过临床预测试进行量表调试,最终确定OCS-P。选取220例福建中医药大学附属康复医院的住院、门诊脑卒中患者和福州市社区认知功能正常者作为研究对象,其中,脑卒中组纳入100例;正常人对照组根据年龄分为青年组(25~35岁)和中老年组(40~70岁),各纳入60例。根据受试者的OCS-P得分,检验其结构效度、实质效度和同时效度、评定员间信度、重测信度和内部一致性信度,以及区分脑卒中和健康人群认知差异的能力。结果:1.通过专家评审形式的内容效度检验,对OCS-P图片命名、定向力和句子朗读及延迟回忆和识别的10个条目进行了修改,通过预测试对句子朗读进一步修改,最终形成符合中国大陆文化特色和语言特点的OCS-P。2.OCS-P具有较为合理的效度。探索性因素分析结果显示量表由三个因子组成,且总方差解释率为60.7%;实质效度的难度等级为0.59~0.97,区分指数为0.09~0.65;同时效度检验显示OCS-P与中文北京版蒙特利尔认知评估量表(Beijing Version of Montreal Cognitive Assessment, MoCA-BJ)在总分水平的Pearson相关系数为0.81(P<0.001),在分测试水平的Pearson相关系数为0.45~0.79(P<0.001),OCS-P的实践测试与Goldenberg测试(Goldenberg's test)的Pearson相关系数为0.72(P<0.001)。3. OCS-P信度良好。总分水平的评定员间信度组内相关系数(intraclass correlation coefficient, ICC)为0.90,分测试水平的ICC为0.74~0.99;总分水平的重测信度ICC为0.68,大部分分测试水平的ICC为0.54~0.95;内部一致性信度克朗巴赫α系数(Cronbach'sα)为0.69;除了视野检查和划心测试中的空间不对称之外,脑卒中组在分测试水平的得分显著低于中老年对照组(P<0.005)。结论:1.经翻译和修订形成的OCS-P是一项适合中国脑卒中患者的简易认知筛查量表。2.OCS-P在我国脑卒中患者人群中具备良好的信度和效度,并且能够有效地区分脑卒中患者和健康人群的认知差异,可以作为临床脑卒中患者认知功能的筛查工具。
