丹参、黄芪和葛根等配伍抗大鼠脑I/R损伤及血栓形成的作用研究

摘要
缺血性心脑血管疾病为致死致残率均很高的常见病,其治疗过程中造成的脑缺血/再灌注(Ischemia/Reperfusion, I/R)损伤更是严重危害人类健康,而血栓形成与诸多缺血性心脑血管疾病密切相关,是此类疾病的主要病因之一。中药能够通过多环节、多靶点对此类疾病进行防治。单味中药丹参、黄芪和葛根等常被用于治疗心脑血管疾病的中药组方中,有良好的临床疗效,但其配伍抗脑I/R损伤和血栓形成作用目前未见报道。采用MCAO法制作大鼠局灶性脑I/R损伤模型,通过神经功能障碍评分、TTC染色、Nissl染色等方法综合评价丹参、黄芪和葛根等配伍对此模型大鼠的神经保护作用;并通过MDA、CAT、SOD、iNOS和TNOS等生化指标的测定、放射性免疫法、免疫组织化学技术和蛋白质印迹法等探讨此组方对大鼠脑I/R损伤的保护机制。研究发现此组方可显著改善神经功能障碍,减少大鼠大脑梗死体积,增加神经元存活数目,增强SOD、CAT活性,减少MDA含量,降低iNOS及TNOS活性,减少C aspase-3活性细胞数目,下调Bax蛋白的表达,上调Bcl-2蛋白的表达,降低Bax/Bcl-2,表明丹参、黄芪、葛根等配伍对脑I/R造成的损伤有保护作用,作用机制与抗细胞凋亡和抗氧化有关。采用尾静脉注射去甲肾上腺素和牛血清白蛋白,并结合冰水浴复制大鼠血瘀证模型,用直流电刺激颈动脉,测定血栓湿重、长度、出凝血时间、抗凝活性、相关血液生化指标,评价丹参、黄芪和葛根等配伍的抗血栓作用,并初步探讨其机制。结果表明此配伍能够降低血栓湿重,缩短血栓长度,延长PT、APTT、TT,降低FIB,下调TXB2和ET水平,上调6-keto-PGF1α和eNOS水平。表明此配伍可明显预防血栓形成,且其机制可能与其抗凝活性、调节血管内皮活性因子释放及维持TX KA2和PGI2平衡有关。研究表明,丹参、黄芪和葛根等配伍有保护大鼠局灶性脑I/R损伤和抑制血栓生成作用,为其用于心脑血管疾病治疗提供理论基础。
