不同强度吸气肌训练在脑卒中偏瘫患者中的应用研究

摘要
研究目的:本研究旨在探讨不同强度吸气肌训练对脑卒中偏瘫患者吸气肌功能、肺功能、运动功能、日常生活能力的干预效果;同时验证高强度吸气肌训练在脑卒中偏瘫患者中干预效果的安全性和有效性。研究方法:以2020年1月至2021年1月浙江省湖州市某三级医院康复科脑卒中偏瘫患者78例为研究对象,根据随机数字表法将患者分为对照组(A组)、中强度组(B组)、高强度组(C组),每组26人。三组患者均接受康复科常规治疗、常规康复及常规护理,B、C组在此基础上分别实施中强度和高强度吸气肌训练,每天1次,每周5-7天,共持续干预6周。分别于干预前、干预第2周、4周、6周采用重复测量方差分析对三组研究对象最大吸气压(Maximum Inspiratory Pressure,MIP)、吸气流速峰值(Peak inspiratory flow,PIF)、用力肺活量(Forced vital capacity,FVC)、第一秒用力呼气容积(Forced expiratory volume in one second,FEV1)进行评估;于干预前、干预6周后采用单因素方差分析对三组研究对象Fugl-Meyer运动评分(Fugl-Meyer Assessment,FMA)、改良Barthel指数(Modified Barthel Index,MBI)进行评估。研究结果:(1)本研究干预过程中失访2例,最终纳入76例患者,其中A组26例,B组和C组各25例,三组研究对象的年龄、性别、脑卒中类型、偏瘫侧、神经功能缺损评分等一般资料,经比较差异无统计学意义(P>0.05)。三组患者在干预期间均未发生不良事件。(2)吸气肌功能:干预前三组患者MIP、PIF值经比较差异无统计学意义(P>0.05)。组内比较:A组患者干预2周、4周、6周与干预前相比,差异无统计学意义(P>0.05);B组、C组患者干预4周、6周与干预前相比,差异有统计学意义(P<0.05)。组间比较:干预2周时,三组患者MIP、PIF值经比较差异无统计学意义(P>0.05);干预4周时,三组患者MIP、PIF值经比较差异有统计学意义(P<0.05),B组和C组的MIP、PIF值改善效果高于A组;干预6周时,三组间MIP、PIF值比较差异有统计学意义(P<0.05),C组改善程度高于B组和A组。(3)肺功能:干预前三组患者FVC、FEV1值经比较差异无统计学意义(P>0.05)。组内比较:A组患者干预2周、4周、6周与干预前相比,FVC、FEV1差异无统计学意义(P>0.05);B组、C组患者干预4周、6周与干预前相比,差异有统计学意义(P<0.05)。组间比较:干预2周时,三组患者FVC、FEV1值经比较差异无统计学意义(P>0.05);干预4周时,三组患者FVC、FEV1值经比较差异有统计学意义(P<0.05),其中B组和C组的分值高于A组;干预6周时,三组间FVC、FEV1值经比较差异有统计学意义(P<0.05),其中C组分值高于B组和A组。(4)运动功能:干预6周后三组患者FMA评分均较干预前提高,差异具有统计学意义(F=5.518,P<0.05),B组和C组的分值高于A组(P<0.05)。(5)日常生活能力:干预6周后三组患者MBI评分均较干预前提高,差异具有统计学意义(F=6.662,P<0.05),B组和C组的分值高于A组(P<0.05)。研究结论:(1)吸气肌训练可以改善脑卒中偏瘫患者吸气肌功能、肺功能,运动功能、日常生活能力。(2)高强度吸气肌训练对脑卒中偏瘫患者吸气肌功能、肺功能改善效果更佳,且具有安全性和有效性。
