基于ERK-CREB-BDNF通路探讨解郁宁神汤对郁火脾虚PSD大鼠的作用机制

摘要
目的为了提高卒中后抑郁(Post-stroke depression,PSD)的临床治疗率,特探讨中药复方解郁宁神汤对郁火脾虚型PSD大鼠的作用机制,为临床治疗提供客观的实验室数据支撑。方法实验一:140只SD雄性大鼠(200±20g)分为空白组(Control)、卒中组(MCAO)、模型组(PSD)、氟西汀组(Flu)、解郁宁神汤高剂量组(JYNS-H)、解郁宁神汤中剂量组(JYNS-M)、解郁宁神汤低剂量组(JYNS-L)。空白组不行任何处理;卒中组仅行MCAO术;模型组、氟西汀组、解郁宁神汤(高中低)剂量组于MCAO术后实施4周慢性不可预知刺激(Chronic unpredictable mild stimulation,CUMS)包括:(1)禁食、禁水24h;(2)昼夜颠倒24h;(3)晃笼(200Hz 5min);(4)湿笼100m L水洒在笼子里24h;(5)夹尾2 min;(6)电击足底(0.9m A,15s×4次);(7)强迫游泳试验(4℃5min)。结束后给予氟西汀或解郁宁神汤高、中、低剂量灌胃4周。实验第4、8周行尿木糖排泄率测定、体重测量、强迫游泳实验、糖水消耗实验、新奇抑制摄食实验、敞箱实验的行为学评估。实验二:行为学评估、灌胃结束后取脑组织行皮层HE染色。实验三:Western blot检测大鼠海马与皮层ERK、p-ERK、CREB、p-CREB、BDNF的蛋白表达,ELisa检测大鼠海马与皮层的BDNF含量。实验四:60只SD雄性大鼠(200±20g)随机分三组(n=20)。分别行大脑中动脉栓塞(Middle cerebral artery occlusion,MCAO)30min、60min、90min,术后24h进行神经功能缺损评分,各组随机抽取3只行脑组织TTC染色,评估脑梗死体积,剩余大鼠评估14天存活率。结果实验一:行为学结果(1)实验第4周,与空白组对比,模型组、氟西汀组及解郁宁神汤组大鼠尿木糖排泄率显著降低(P<0.05),同时,各组大鼠体质量、强迫游泳不动时间、糖水消耗量、新奇抑制摄食潜伏时间、敞箱实验得分结果与空白组对比差异显著(P<0.01),建模符合郁火脾虚型PSD大鼠。(2)实验第8周,与空白组相比,模型组大鼠行为学指标差异显著(P<0.01)。与模型组相比,氟西汀组和解郁宁神汤高剂量组各项行为学测试改善明显(P<0.01),解郁宁神汤中剂量组上述行为学变化存在差异(P<0.05),解郁宁神汤低剂量组有改善趋势,但无统计学差异(P>0.05)。实验二:病理组织形态学HE染色后,PSD组大鼠皮层细胞排列疏松,空泡状细胞多,细胞核固缩、偏移。氟西汀组及解郁宁神汤高、中剂量组皮层细胞排列紧密,较为整齐,细胞核数目增多。实验三:Western-blot检测p-ERK/ERK、p-CREB/CREB和BDNF:与模型组相比,解郁宁神汤高剂量组(P<0.01)及中剂量组(P<0.05)均能上调p-ERK/ERK、p-CREB/CREB和BDNF蛋白表达,氟西汀组可提高p-ERK/ERK、BDNF蛋白表达(P<0.05),但对p-CREB/CREB蛋白表达无影响。Elisa显示氟西汀组及解郁宁神汤高、中剂量组海马及皮层BDNF含量均有升高(P<0.05)。实验四:卒中后抑郁模型大鼠的改良30min组的大鼠生存率为80%,但神经功能缺损评分及脑梗死体积均不能达到卒中模型建立的标准;与30min组对比,60min和90min组神经功能缺损评分均大于1分,且脑梗死体积存在统计学差异(P<0.05);90min组大鼠生存率仅有30%,不能为后续实验提供保障;因此,60min为卒中后抑郁模型大鼠脑缺血再灌注的最佳时间点。结论1.解郁宁神汤高、中剂量可明显改善大鼠抑郁行为,与氟西汀组效果相当;解郁宁神汤促神经元存活,可能通过ERK-CREB-BDNF信号通路,发挥抗抑郁作用;2.卒中后抑郁模型构建中大鼠脑缺血再灌注的最佳时间点为60min,既可以模拟脑梗死稳定的病理改变,提高生存率,又可以为后续抑郁模型的叠加提供保障。
