摘要：

目的系统评价早期循序渐进运动(early progressive exercise,EPE)对ICU获得性衰弱的影响。方法检索The Cochrane Library、The Joanna Briggs Institute Library(JBI)、Pub Med、MEDLINE、EMBASE、CINAHL、中国生物医学文献数据库、中国知网及万方数据库中关于EPE对ICU获得性衰弱影响的随机对照试验。经筛选文献,提取资料与评价文献质量后,采用Rev Man 5.3软件行Meta分析。结果最终纳入11项随机对照试验,1378例患者。经Meta分析,EPE能有效提高患者的医学研究理事会评分[Z=6.19,95%CI(4.15,8.00),P<0.001],改善患者的独立功能状态[Z=2.48,95%CI(1.45,23.88),P=0.01],但EPE在增强患者肌力强度、最大吸气压力方面并未显示明显优势,合并效应无统计学意义(P>0.05)。结论 EPE可有效改善ICU机械通气患者独立功能状态,但在预防ICU获得性衰弱方面并未显示出优势,尚需开展深入研究进行验证。
