类风湿关节炎昼夜节律异常与时间治疗研究进展

摘要：

昼夜节律维持着机体的内在协调与外部统一,普遍存在于哺乳动物中,维持着正常的生理功能。类风湿关节炎(RA)已被证实昼夜节律存在异常,其关节肿痛、僵硬等临床症状晨起加重的临床现象被认为与RA患者体内的炎性因子如白细胞介素(IL)-6、IL-1、IL-2、IL-12、肿瘤坏死因子α(TNF-α)等在夜间异常升高,而具有抗炎作用的皮质醇等激素分泌相对不足有关。下丘脑-垂体-肾上腺轴损伤、褪黑素与脂肪因子的水平波动可能在造成RA昼夜节律异常中起重要作用。时间治疗有助于优化RA治疗方案,睡前给予小剂量糖皮质激素能更好地控制炎症,改善晨僵症状,且不影响人体自身下丘脑-垂体-肾上腺轴的功能。非甾体类消炎药、慢作用药物的时间治疗相关研究较少,尚需要更高级别的证据。
