Urinary tract infections in women aged 15 to 65 years in open-care practice:: monitoring of Escherichia coli sensitivity to fosfomycin trometamol on the basis of the previous history.

The objective of this study was to re-evaluate the sensitivity of Escherichia coli to antibiotics used in open-care practice, particularly fosfomycin trometamol, and to analyze the influence of epidemiological resistance factors (history of hospitalization, urinary tract infection, antibiotic therapy and urinary catheterization). This study was conducted in France in nine open-care medical laboratories (Aforcopi-Bio) from January to March 2000. Study of 461 specimens confirmed the predominance of Escherichia coli (79.8%) in urinary tract infections in women aged 15 to 65 years. The sensitivity of E. coli to fosfomycin has remained high (98.1%) and constant, compared to the previous decade and the other antibiotics: amoxicilline (54%), amoxicilline-acide clavulanique (65,5%), cotrimoxazole (77,8%), nitrofurantoine (89,3%), acide nalidixique (91,6%), norfloxacine (93,9%) and ciprofloxacine (95%). In contrast to the results for quinolones and third-generation cephalosporins, sensitivity to fosfomycin trometamol was not influenced by recent antibiotic therapy or previous episodes of urinary tract infection.
