55~70岁女性肌肉质量与力量增龄性特征实验研究

摘要
研究目的:衰老是不可抗拒的生理过程,由此引发的无力、骨折、免疫力下降和活动能力降低等危害。增龄性的肌力流失作为衰老的重要表现,是一种随着年龄增长肌肉质量、肌肉力量和活动能力普遍下降的症候群,影响老年人的日常生活。肌力流失的研究多集中发病率、肌肉质量或肌肉力量与运动功能的关系等方面,对于肌肉质量和力量的增龄性变化特点及不同部位流失速率的研究较少且尚无一致结论。本研究通过对青年和中老年女性的肌肉质量和肌肉力量进行测量,尝试阐述肌肉质量与力量的流失特征与不同部位的肌力流失速率,为预防老年人肌力流失提供理论参考。研究方法:本研究共招募114名女性受试者,受试者分为四组,青年组(n=29)、56~60岁组(n=28)、61~65岁组(n=28)和66~70岁组(n=29)。使用双能X线对受试者进行全身肌肉含量扫描,获得上肢、躯干、下肢和全身瘦体重。使用等速肌力测试仪对下肢髋关节、膝关节和踝关节进行等长和60°/S等速肌肉力量测量,获取不同速度下的三关节屈肌和伸肌最大平均肌肉力矩和平均功率。通过计算获得以青年组作为标准化的瘦体重比和肌肉力量比。研究结果:1.瘦体重结果左侧、右侧和双侧的下肢与身体总瘦体重在四组间存在明显差异(P<0.01),瘦体重的流失主要发生在下肢。青年组的下肢和身体总瘦体重分别与三组中老年组存在统计学差异(P<0.05),而三组中老年组的组内比较均无明显差异存在,瘦体重至少从56~60岁后发生明显流失。瘦体重比结果显示下肢的瘦体重流失明显多于上肢、下肢。2.等长肌肉力量结果髋关节屈曲和伸展、膝关节屈曲、踝关节伸展力矩在四组间存在统计学差异(P<0.01);事后比较及肌肉力量比结果发现,髋关节屈曲和伸展力矩是至少从56~60岁后发生明显流失,在56~70岁年龄段中,61~65岁之后发生流失速率加快;膝关节屈曲力矩至少从61~65岁后发生明显衰退,61~65岁之后发生流失速率加快;踝关节伸展力矩至少从56~60岁后发生明显流失,其肌肉力量流失较为平均。3.等速肌肉力量结果髋关节屈曲、膝关节屈曲和伸展、踝关节屈曲和伸展力矩在四组之间存在统计学差异(P<0.01);髋关节屈曲和伸展、膝关节和伸展和踝关节屈曲和伸展平均力矩存在统计学差异(P<0.01)。事后比较及肌肉力量比结果发现,髋关节屈曲力矩是至少从56~60岁后发生明显流失,在56~70岁年龄段中,61~65岁之后发生流失速率加快;膝关节屈曲力矩至少从61~65岁后发生明显衰退,膝关节伸展力矩至少从56~60岁后发生明显衰退,两者在56~60至61~65岁之间发生流失速率更快;踝关节屈曲和伸展力矩至少从56~60岁后发生明显流失,两者在56~60至61~65岁之间发生流失速率更快。研究结论:1.肌肉质量流失(1)相比于躯干和上肢肌肉质量,下肢的肌肉质量的流失出现时间较早。(2)下肢肌肉质量增龄性变化中,60岁之后出现加速流失趋势。2.肌肉力量流失(1)踝关节伸展的等长肌肉力量较屈曲力量流失更早且流失速率更快。膝关节伸展等速肌肉力量比踝关节和髋关节流失更早。踝关节伸展和膝关节伸展分别表现出关节肌力流失的不均衡性。(2)屈曲功能链(髋关节屈曲、膝关节屈曲和踝关节伸展)在等长和等速肌肉力量较之拮抗肌的肌力流失明显。
