影像学定量评估腰椎间盘退变的研究进展

摘要
<正>腰痛是临床上最为常见的疾患之一,常反复发作,严重影响患者的生活质量[1]。椎间盘退变(intervertebral disc degeneration,IDD)是引起腰痛最常见的病因[1~3]。IDD在早期干预治疗效果最佳,当椎间盘退变程度较重时,椎间盘修复的可能性极低[2、4]。常规影像学检查难以对IDD早期进行检测和定量评估[4~6]。在一些关于IDD再生修复治疗的新手段的临床研究中,多以患者的疼痛改善、椎旁肌肉肌电信号、椎间盘高度、髓核视觉信号改变等作为疗效评估指标[2、7]。这些指标无法直接定量反映退变椎间盘内细胞外基质的增减,难以准确客观地说明退变椎间盘的再生修复情况。因此,早期诊断和定量评估IDD的意义重大。
