钽金属骨小梁臼杯联合钽金属加强块重建髋臼严重骨缺损的临床研究

摘要
目的探讨联合应用钽金属骨小梁(TM)臼杯联合钽金属加强块重建髋臼严重骨缺损的早期临床效果。方法回顾性分析北京积水潭医院矫形骨科2014年4月至2015年4月,在全髋关节置换术中联合应用TM臼杯和钽金属加强块重建髋臼严重骨缺损患者19例20髋,其中男8例9髋,女11例11髋。所有患者均得到早期随访,平均随访8.4(4~17)个月,平均52.7(28~62)岁。均采用配对t检验统计术前及随访时髋关节功能评分(Harris评分);测量髋关节旋转中心相对于泪滴连线位置;采用X线片观察术后即刻及随访时TM臼杯及钽金属加强块周围透亮线、假体是否移位;统计所有并发症情况。结果髋关节手术前后的Harris总分术前34.0分,随访时78.3分,手术前后Harris总评分具有统计学差异(t=-17.01,P<0.01)。患侧旋转中心高度术前34.4 mm,术后21.4 mm,手术前后旋转中心的高度具有统计学差异(t=-3.11,P<0.01)。所有患者随访时臼杯及钽金属加强块均稳定。所有患者随访时无感染、脱位、假体周围骨折等并发症。结论 TM臼杯联合钽金属加强块重建髋臼严重骨缺损能够增加与宿主骨的接触面积,获得良好的初始稳定性;同时又能重建相对正常的旋转中心,为恢复正常的髋关节生物力学打下基础。
