热湿交换器和加热湿化器对呼吸机相关性肺炎的预防价值

摘要：

目的比较热湿交换器和加热湿化器对呼吸机相关性肺炎(VAP)的预防作用。方法检索PubMed等数据库,查找所有对比使用热湿交换器和加热湿化器患者VAP发病率的随机对照研究,应用Review Manager 5.0分析软件,对所选文献结果进行综合分析。结果共纳入15项随机对照研究。对于降低VAP发病率:热湿交换器和加热湿化器相似(OR1.18,95%CI[0.96,1.44]),但热湿交换器优于未含管道加热导丝的加热湿化器(OR1.39,95%CI[1.08,1.79]);使用热湿交换器和加热湿化器的患者死亡率、机械通气时间、ICU住院时间相似。结论预防VAP热湿交换器优于未含管道加热导丝的加热湿化器,但并不优于含管道加热导丝的加热湿化器。
